NAFI Connecticut, Inc. (NAFI CT) is one of Connecticut’s most respected child and family serving agencies, with program locations across the state. We are seeking a Part-Time Registered Nurse (RN), approximately 20 hours per week, to join a short-term residential program providing compassionate, trauma-informed care for young women ages 12–17. The program provides comprehensive clinical, therapeutic, educational, medical, and case management supports designed to promote each young person’s health, safety, stability, and well-being while supporting successful transitions to family, caregivers, or other appropriate community settings.

The Part-Time RN will work collaboratively with the program’s Full-Time Nurse and Medical Services Coordinator, as well as clinical, case management, educational, and residential staff, to ensure that youth receive coordinated, high-quality medical care and support. At NAFI CT, you’ll be part of a team of compassionate, skilled, solutions-oriented professionals who are unwavering in their commitment to the children and families in our care.

Responsibilities Include:

  • Work collaboratively with the Full-Time Nurse and Medical Services Coordinator to support the medical and health care needs of youth in the program
  • Complete nursing assessments and health screenings, including assessments completed as youth enter the program
  • Triage medical concerns and provide nursing guidance and follow-up as appropriate
  • Assist with coordinating appointments and follow-up for off-site medical services, including primary care, dental, vision, specialty, and other health care services
  • Maintain thorough and accurate electronic health records in accordance with applicable laws, agency policies, and funder or regulatory requirements
  • Support safe and effective medication management practices, including medication reconciliation and review of documentation for accuracy and compliance
  • Collaborate with youth, families, caregivers, and community providers to promote continuity of care and successful transitions from the program
  • Provide health education and guidance to youth, families, caregivers, and staff regarding medical needs, medications, wellness, and follow-up care
  • Develop and maintain positive and productive working relationships with health care providers, funding sources, collaborative partners, and other stakeholders
