Job Responsibilities:

• Identify, plan, coordinate community-based services for Clients.
• In-service training for CNA/Aides.
• Conduct in-home visits and assessments.
• Maintain Client records.
• Conduct skilled service for clients in home:
  Wound care/ dressing changes Skin Monitoring Tube Feeding Ostomy and catheter care Medication Management Insulin/ syringe management Bowel Management Client/ Family training

Qualifications:

• At least 3 years' experience in the health and human services field.
• Active RN license, with at least 3 years of experience in Home Health Care.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.
• Good communication and customer service skills.
• Excellent organizational and decision-making.
• Valid Driver's License.
• CPR/First Aid.
• Current TB.
