Certified Nursing Assistant; CNA

About the Role

As a Certified Nursing Assistant, you will provide compassionate care during patients’ final days, collaborating closely with an RN Case Manager. You’ll perform personal care activities, advance your skills through structured training, and enjoy reasonable caseloads and on‑call support in a supportive environment.

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem‑solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver’s license (required)
• Valid auto insurance (required)

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range‑of‑motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
