Position Summary
The Transition Academy Coordinator supports Student Specialists and Life Skills Coaches with students and their planned goals, recreational activities, vocational opportunities, life skills and personal care. The Coordinator works with staff and students to improve independence through support of weekly goals, teaching life skills, vocational skills, social skills, healthy habits, budgeting, meal planning, cooking assistance, and other independence‑building skills.
Position Qualifications- Preferred:
Bachelor's degree in psychology, education, special education, sociology, social work, or a related field - Preferred: RBT Certification
- Required:
Completion of company training within the first 30 days (First Aid, CPR, Safety Care, Medication Delegation, HIPAA, ANE) - Required:
Successful criminal background check and drug test - Required:
Valid Texas driver’s license with an acceptable driving record - Required working conditions:
Indoor (office, house, classroom) and outdoor environments with climate variations; strength/endurance requirements (lifting, carrying, pushing, pulling); exposure to injury hazards; physical ability to respond to first aid or CPR situations; and ability to assist adults in emergency situations
- Meet with the TA Director for student planning and professional development
- Assist classroom instructors with behaviors, accommodations, and learning
- Run small‑group lessons with students
- Attend weekly meetings with Life Skills Coaches and Student Specialists for updates and collaboration as needed
- Help with onboarding for new hires through overlapping training sessions and assistance in 29 Acres 101 leadership
- Provide support to staff in daily life‑skills training operations, including activity planning, vocational training, budgeting, cooking, social programs, and evening/weekend staff coordination
- Oversee timecard approvals and corrections on a bi‑weekly basis (Freshman TA Coordinator); oversee the TA extension program (Sophomore)
- Meet weekly with Student Specialists to ensure progress toward set program goals and support self‑determined living based on programming and personal goals
- Assist students with medication refills, scheduling personal appointments, and arranging transportation
- Assist students and staff in finding jobs, scheduling interviews, preparing for employment, and attending employment with the Vocational Services Team
- Collect data, observe, and evaluate as directed toward goals
- Ensure safety and welfare, including medication administration and first aid or CPR when necessary
- Serve as an advocate for students
- Engage in ongoing professional development and staff training
- Collaborate with other professionals while maintaining a proactive, “can do” attitude for all situations
- Perform other assignments as delegated by the Director of the Transition Academy
- Work with the Director and direct staff proactively on student problem behavior until resolved
- Cultivate a collaborative and supportive culture among Student Specialists, Life Skill Coaches, and other staff, fostering open communication and teamwork
- Advocate for a client‑centered culture, ensuring guidance aligns with individualized plans for skill development and independence
- Encourage a positive and inclusive atmosphere, recognizing and valuing diverse strengths and backgrounds of staff and students
- Promote continuous learning by providing mentorship and support for Student Specialists and Coaches to enhance effectiveness in supporting student goals
- Uphold ethical standards and confidentiality in providing assistance for life skills, vocational skills, and personal care, ensuring student well‑being
- Strong leadership qualities, providing guidance and mentorship
- Excellent communication and interpersonal skills, facilitating effective interactions among staff and students
- Compassion and empathy, recognizing and understanding the unique needs and progress of each student; support Student Specialists and Life Skill Coaches
