Summary: Serves as the first introduction to hospice and palliative care for a hospitalized patient and their family. The Hospice Post Acute Liaison educates the patient and their families on Hospice care and provides ongoing support. This person triages all incoming referrals and coordinates interdisciplinary team members required to best meet the immediate needs of the patient. Together with the patient, family and physician, the Clinical Liaison aids in developing a care plan according to their wishes.
In addition, he/she will initiate the assessment to ensure that the care plan is appropriate and is followed.
- A valid and unrestricted Registered Nurse (RN) license in state of practice
- Current driver’s license
- Current CPR certification recognized by Baptist Health
- A strong clinical background is essential. One to two years of nursing hospice experience is preferred
This job will be authorized 80.00 hours bi-weekly.
