Uses knowledge, skill, and experience to analyze, think critically, plan, prioritize, and organize care, make appropriate clinical judgments and efficiently manage routine and unexpected clinical situations.

Required Education

Associate's degree or equivalent from two-year college program or technical school or nursing diploma

Demonstrated performance of 100% at the previous level and score within documented RN II/III levels via ACHieve career ladder for internal employees., Nursing - 1 year of experience

Description
  • Demonstrates and promotes excellence in the application of the AC Model of Patient and Family-Centered care. Collaborates and role models partnerships with patients, families and other providers to plan, deliver, and evaluate highly individualized care that ensures achievement of effective self-care by patients and families. Consistently plans and delivers high quality, comprehensive and holistic care for complex patient situations. Anticipates needs across the care continuum for patients and families and acts as a resource to others in doing the same.
  • Consistently provides safe and effective nursing care at a proficient level. Role models and promotes practice which is current and consistently in accordance with policies and procedures, professional practice standards, state Practice Act/Rules and Regulations. Demonstrates proficient technical and procedural nursing skills. Role models and promotes effective interdisciplinary communication to assure continuity of care to achieve desired patient outcomes.
  • Proficiently assesses increasingly complex patients and serves as a resource to others to ensure developmentally appropriate and culturally sensitive care. Anticipates, incorporates, and promotes the use of appropriate diversional, play, or recreational activities into patient care.
  • Analyzes increasingly complex patient/family information to identify problems and outcomes, evaluates complex variations and patterns in patient care data for clinical implications. Develops, maintains, and promotes active use of the plan of care to ensure that desired patient/family outcomes are achieved. Collaborates with the patient, family, and interdisciplinary team members for ongoing plans. Identifies and competently and promptly responds to changes in increasingly complex patient condition.

    Acts as a resource and guides others in appropriate responses to changes in patient conditions.
  • Provides individualized patient and family education at proficient levels in increasingly complex care situations and serves as a resource in guiding others. Proficiently assesses patient and family learning needs and develops appropriate individualized education plans. Assesses completeness of all patient and family education and recognizes additional needs in comprehensive interdisciplinary teaching.
  • Advances evidence-based nursing practice using best available evidence, patient's and family's preferences and values, and team's clinical expertise at a proficient level. Collaborates with others to search for and apply evidence to guide clinical practice.
  • Exemplifies, supports and serves as a resource for understanding the AC Professional Practice Model. Evaluates own performance to identify areas of strength and needs for professional development. Provides input for peer evaluations and mentors others.
