Part Time RN

• Provide direct patient care as an RN.
• Coordinate care for patients and triage patients.
• Work collaboratively with clinic providers, fellows, and staff.
• Facilitate appointments, coordinate transitions of care, and provide patient education on complex conditions.
• Develop and implement individual health plans in collaboration with patients, their families, and providers of care.
• Perform comprehensive nursing assessments and identify patient health needs.
• Develop and implement individualized care plans in collaboration with physicians, nurse practitioners, and other healthcare providers.
• Coordinate patient care, including referrals, follow-up appointments, diagnostic testing, and continuity of care.
• Educate patients and families on disease prevention, health maintenance, medications, treatment plans, and self-management strategies.
• Monitor patient progress, document assessments and interventions in electronic medical record, and communicate changes in patient status to providers.

• Baccalaureate Degree BSN, ADN Degree, or Diploma RN. Major:
  Registered Nurse.
• RN with a minimum of 1 year of RN experience.
• Current BLS certification.
• ACLS certification or obtain certification within one year of hire date preferred.
• Knowledge or experience in PCMH desired.

Demonstrates expertise in providing direct patient care, developing and implementing individualized care plans, and coordinating comprehensive patient care in collaboration with healthcare providers. Proficient in patient education, health assessments, and electronic medical record documentation.
