Summary of

Job Duties:

This part-time Registered Nurse (RN) position averages between 20 and 30 hours per week and is benefits-eligible. This position is ideal for a nurse seeking consistent part-time hours and a supportive team environment.

The Registered Nurse will promote wellness; improve outcomes, and self-care within a defined population of patients. The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management.

Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes.

Qualifications:

Minimum Qualifications:

  • Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing ;
  • Valid RN License;
  • Minimum of 1 year Registered Nursing experience.

Preferred Qualifications:

  • 3 years of RN experience with current BLS certification.
  • Knowledge or experience in PCMH
  • ACLS certification or obtain certification within one year of hire date.

Additional Information:

BENEFITS ELIGIBLE - This position offers part-time hours ranging from 20 to 30 hours per week, with a schedule that includes daytime shifts. There are no night, weekend, or most State holiday commitments associated with this role.
