Requirements:

• Current Nursing License in the Practicing State
• Valid Driver's License
• Current CPR Certification
• Two years experience as an LPN in a clinical setting, preferably in a home health or hospice setting
• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Ability to use electronic medical charting and time-keeping software
• Reliable transportation and current auto liability insurance coverage
• Must read, write and speak fluent English

Responsibilities:

• Administer skilled nursing care to patients as prescribed by the physician.
• Assist in planning, coordinating, and implementing patient care in conjunction with the supervising RN.
• Deliver effective communication to patients, families, and healthcare professionals.
• Help patients and families learn self-care techniques.
• Utilize infection control measures to protect staff and patients.
• Assist with end-of-life care for hospice patients as needed.
• Participate in admitting, discharging, and transferring patients.
• Engage in annual in-service and performance improvement programs.
