Therapeutic Family Worker

A. Coordinates Client Case Management Activities

1. Assists with referral and admission procedures

2. Secures intake information, e.g., social medical, educational history and completes required forms

3. Assists in the development of service plans for clients under supervision and direction of professional

4. Coordinates activities and makes referrals to assure appropriate and needed services are provided

5. Serves as client advocate with court, school, DCFS and other agencies or services

6. Documents all activities and maintains progress reports, All documentation is completed and submitted within required timeline

7. Participates in regular staffings to review case and revise service plan as needed

8. Serves as liaison between The Centers and community resources

9. Assists in the development and implementation of behavior management programs

10. Develops a schedule for weekly or monthly staffing and staffing aftercare/discharge plan as required

11. Compiles and completes monthly reports

12. Confers with outside agency staff concerning aftercare planning

13. Maintains an active caseload of clients and monitors clients

14. Monitors the placement of foster children in therapeutic foster homes.

B. Documentation

1. Ensures the client record accurately reflects the intervention taking place, the strategies delineated in the treatment plan are carried out and there is a logical flow to the intervention that has taken place based on the clinical information.

2. Documentation is completed according to timelines and format delineated in the policy and procedures manuals for each type of client record entry. Documentation is written such The Centers personnel, third party reviewers and future providers can discern the plan, course and effectiveness of treatment.

3. Logs all relevant phone calls in the progress notes delineating date and purpose of call.

C. Provides Guidance And Support To Client

1. Drives agency and/or own vehicle to transport clients to appointments, clinic, school, court, etc.

2. Offers on-going support and guidance to individual clients and families through telephone contacts, on/off site and crisis intervention, and collateral services

3. Serves as a role model to client

4. Assists in the development and implementation of individualized behavior programs for clients.

D. Productivity

1. Reaches Billable Hours of per month as defined by the attached work plan

2. Maintain and manage caseload as assigned.

Education and/or

Experience:

Bachelor's Degree in Psychology, Sociology, Social Work or related field

At least 2 years experience (direct) working with clients in therapeutic foster care preferred

Must have reliable transportation in order to transport clients should the need arise

Valid Arkansas driver's license with record acceptable to insurance carrier
