Adaptive Home Health is seeking a Licensed Vocational Nurse (LVN) to deliver skilled nursing visits in patients’ homes. The field clinician works under RN/clinical supervision with a focus on timely assessments, escalation when needed, and accurate charting in Homecare Homebase (HCHB).
You will support medication reconciliation, patient education, and wound care, while coordinating with the office to ensure on-time visits and compliance with the plan of care.
