Job Description:

Performs initial evaluations with patients and families. Assists with consultations and interventions for chronic disease management. Collaborates with physicians and healthcare team members. Develops and implements interdisciplinary treatment plans. Assists with advance care planning. Modifies care plans as needed. Facilitates decision-making regarding treatment. Works with healthcare professionals and community agencies. Communicates effectively with patients and families. Responds to patient needs. Maintains accurate patient records.

Provides education related to palliative care. Other duties as assigned.

  • Ability to communicate effectively with diverse populations. [Required]

  • Critical thinking and problem solving skills. [Required]
  • Strong clinical assessment skills. [Required]
  • Outstanding interpersonal, communication, group facilitation, counselling and organizational skills. [Preferred]
  • Significant knowledge and understanding of availability of community and post-acute resources. [Preferred]
Education:
  • Associate [Required]
  • Bachelor's of Nursing [Preferred]
  • Master's of Nursing [Preferred]
Field of Study:
  • Nursing
Work Experience:
  • 1+ year clinical, hospice, or hospital setting [Required]
  • Clinical nursing care experience in palliative care, oncology, hospice, geriatrics, or intensive care [Preferred]
Additional Information:

Licenses and

Certifications:
  • Registered Nurse (RN) [Required]
  • Certified Hospice and Palliative Nurse (CHPN) [Preferred]
  • Basic Life Support - CPR Cert (BLS) [Required]
