Licensed Practical Nurse

Cherrelyn Healthcare Center is seeking a compassionate LPN to join our Healthcare & Medical Services team. In this role, you'll provide direct patient care, administer medications, monitor vital signs, and support residents with daily living activities in collaboration with RNs and physicians. You'll document care accurately, follow individualized care plans, and communicate effectively with residents and families. Candidates must hold a current LPN license and demonstrate strong clinical, organizational, and interpersonal skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration, treatments, and wound care
• Monitor, document, and report changes in residents' conditions to RNs and physicians
• Accurately document assessments, interventions, and outcomes in the electronic medical record (EMR)
• Assist with development and implementation of individualized care plans
• Collaborate with interdisciplinary team members, residents, and families to coordinate care
• Follow infection control, safety, and quality standards to maintain a safe environment
• Support admissions, transfers, and discharges, ensuring smooth care transitions
• Provide education and support to residents and families about health conditions and care plans
• Assist with mentoring and supporting CNAs and new nursing staff as needed
• Participate in quality improvement initiatives and contribute ideas to enhance resident care and satisfaction
