Position: Case Manager – Court-Based Services

Case Manager – Court-Based – All Health Network

At All Health Network, compassion, collaboration, and evidence‑based care come together to support individuals with serious mental illness who are involved in the criminal justice system. Our court‑based case management services work in close partnership with problem‑solving courts and judicial stakeholders to promote treatment engagement, community stability, and improved legal and clinical outcomes.

As a Case Manager
, you are essential in providing coordinated, person‑centered services for this court‑mandated expansion. Your role supports All Health Network’s Intensive Outpatient Services (IOS) Division by filling gaps in care with specialized behavioral health services aligned with court requirements. The position emphasizes care coordination, resource navigation, and community support to ensure ongoing care, encourage recovery, and strengthen partnerships between All Health Network and judicial stakeholders.

What

You’ll Do
  • Maintain an active caseload of justice‑involved clients in accordance with department standards
  • Provide and document case management and care coordination services in compliance with regulatory and contractual requirements
  • Complete comprehensive assessments addressing medical, psychosocial, behavioral health, and community‑based needs
  • Develop, implement, and update multidisciplinary care plans in collaboration with clients and the treatment team
  • Assist clients in identifying recovery goals and coordinating services and providers to support those goals
  • Act as a client advocate and liaison with internal teams, community providers, and criminal justice partners
  • Provide coaching and support services aligned with client readiness to change, including motivational interviewing, skills‑based training, and self‑management support
  • Facilitate referrals to community‑based organizations and resources aligned with identified client needs
  • Work collaboratively within an interdisciplinary, team‑based model, including telephonic and face‑to‑face contacts and community‑based visits
  • Travel as required for hospital visits, provider offices, community organizations, and home visits
  • Promote a positive, collaborative team culture within All Health Network
  • Positively contribute to the organization’s financial health through delivery of billable services and data‑informed program improvement
  • Maintain ethical, professional relationships with clients, community partners, and government agencies
  • Participate in team meetings, huddles, supervision, and training activities
  • Perform case management services consistent with ethical and professional standards
  • Follow all All Health Network policies and procedures
  • Perform other duties within the scope of the position based on experience, education, and ability
What You Bring
  • Bachelor’s degree in a relevant clinical field such as Social Work, Clinical Psychology, or Counseling.
  • Minimum of two (2) years of related clinical experience serving diverse populations in behavioral health, physical health, or criminal justice settings.
  • Demonstrated experience in case management, care coordination, and community‑based services.
  • Strong assessment, care planning, and clinical documentation skills.
  • Working knowledge of community resources, treatment options, funding sources, and support programs.
  • Proficiency with PC‑based software systems, including Word, Excel, PowerPoint, and electronic systems.
  • Excellent written and verbal communication, time management, problem‑solving, and decision‑making skills.
  • Ability to work independently while contributing effectively within a multidisciplinary team environment.
  • Must possess reliable transportation, a valid driver’s license, a good driving record, and proof of insurance
