Contingent upon performance, this position may be eligible for a hybrid schedule and can be structured as either four 10-hour days or five 8-hour days.
- The full salary for the Diversion Case Manager I position is $54,589.08
to $81,831.10 annually. - The full hiring range for the Diversion Case Manager II is $60,009.04 to $90,014.08 annually.
- Additional Diversion Case Manager II Requirements:
- Masters degree in social work (MSW) or closely related human services field.
- Licensure, or candidate for licensure, in a mental health field per Colorado DORA.
- 2+ years of progressively responsible counseling duties involving direct service provision with adults, juveniles, and families.

This position may be eligible for career progression, and we may hire this position at either the Diversion Case Manager I or II level.
GENERAL DESCRIPTION OF JOB:The Diversion Case Manager I provides a variety of support to aid in participant's successful completion of the diversion program including: coordinating both internal and external services, identifying and fostering professional relationships with other community-based providers, connecting diversion participants with community resources and providers as needed, conducting multi-disciplinary meetings, providing educational advocacy, maintaining client records by documenting all participant contacts and tracking progress, and facilitating restorative justice practices in the 18th Judicial District through the District Attorney’s Office.
DUTIES:- Coordinates with the diversion team to identify appropriate restorative justice practices for participants and facilitates restorative justice practices including community circles.
- Collaborates with clinical staff to create individual plans outlining program requirements for both adult and juvenile diversion participants. Ensures appropriate level of supervision for each client, determines frequency and modality of participants contacts with the program.
- Works in conjunction with program participants to promote successful completion of their individualized program goals.
- Coordinates shared treatment planning with external treatment providers, executes appropriate waivers and releases of information, and ensures compliance with treatment and program requirements.
- Researches and identifies community-based resources as needed, conducts due diligence on community-based resources and establishes professional working relationships between agencies to foster ease of access and collaboration.
- Conducts multi-disciplinary meetings. Helps families to understand educational options and coordinates with schools on transitions. Educates client and family regarding the justice system and consequences of further law involvement. Assists in tracking and collection of restitution.
- Creates case management notes in participant electronic files and all other required paperwork or documentation, maintains participant confidentiality, tracks participant progress and helps to determine program violations and successful completion.
- Collects and reports various case-related statistics. May assist with data collection and reporting related to grant reports and applications.
- Formulates case conceptualization and case presentations to supervisor, co-workers and outside agencies.
- Completes screens and assessments to determine risk and need levels and to guide treatment planning.
- Responds to clients in crisis and makes referrals as needed.
