• Work History

• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications - MUST be uploaded with profile (Current ASCP and BLS)

CERTIFICATION REQUIREMENTS :

Weekend Requirements :
As needed/ Must be available for Holiday coverage if needed
On Call Requirements :
