Stratford Villa Post‑Acute is proud to be a Five‑Star, Medicare‑certified, 37‑bed Skilled Nursing Facility located near the heart of downtown Livermore. It is part of the Villas Post‑Acute and Senior Living community for seniors in the heart of Livermore.

Position Overview

The Licensed Vocational Nurse is responsible for assisting the RN’s in providing patient services, organizing, developing, and performing day‑to‑day functions of the Nursing Service Department in accordance with federal, state, and local standards, guidelines and facility policies and procedures, and as directed by the Administrator, the Medical Director, and/or the Director of Nursing Services to ensure the highest degree of quality care is maintained at all times.

Responsibilities
  • Conduct initial and ongoing assessments of residents' health status.
  • Monitor vital signs, administer medications, and observe any changes in condition.
  • Notify the RN or healthcare provider of any significant changes or concerns.
  • Administer medications as prescribed by the healthcare provider.
  • Ensure accurate dosage, proper route, and timely administration.
  • Monitor and record residents' responses to medications.
  • Provide basic wound care and other treatments as ordered by the healthcare provider.
  • Monitor the healing progress of wounds and report any complications.
  • Work closely with Registered Nurses, Nurse Managers, Certified Nursing Assistants (CNAs), and other healthcare professionals to coordinate care.
  • Participate in care planning meetings and contribute to the development of resident care plans.
Qualifications
  • Fresh or newly licensed LVNs are welcome.
  • Experienced LVN is preferred.
  • Experience in a skilled nursing facility/long‑term care/senior care is a plus.
  • Valid and active Licensed Vocational Nurse (LVN) license in the state of California.
  • Must have an active BLS through the American Heart Association.
