Suncrest Hospice - - Responsibilities:
Provide resident care to assigned residents in accordance with resident assessments, care plans and as directed by nursing management;
Coordinate the proper types of care with other licensed care staff;
Work under the direction of a Case Manager/DON;
Collaborate with nurses, social workers, chaplains, volunteers, and STNA leadership;
