Overview

• Conduct adult and geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up and clear documentation according to Matrix guidelines and protocols.
• Administer and order point‑of‑care and lab‑based testing as needed.
• Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
• Collaborate with the primary care physician on patient education and follow‑up as necessary.
• Provide services at the member’s place of residence, which may be in homes or skilled nursing facilities.

• Flexibility:
  Visits are scheduled based on availability from 8 am to 7 pm, Sunday to Saturday, allowing work during the day, evening, or weekend.
• Competitive pay:
  Earn income based on the number of assessments completed; those completing 20‑100 visits per month can earn $1,800‑11,500 per month, with rates of $90‑115 per assessment depending on state.
• Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
• Education and support:
  Comprehensive onboarding, a preceptor, 1:1 feedback, and free continuing‑education access.
• Additional licensure support:
  Assistance obtaining state licensure and credentialing for neighboring or other states as needed.
• Full‑time opportunities: PRN providers can transition to full‑time roles in their home location, state, or region with a sign‑on bonus.

• Master’s degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority as needed.
• Board certification from AANP, AACN, or ANCC in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS, or CPR certification.
• One year of NP experience preferred; new graduates encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage required for the state of residence.
• Strong computer skills and familiarity with employee health/medical record software.
• Excellent verbal and written communication skills.
