Job OverviewThe Advanced Practice Provider is a vital member of our Rapid Response team who works collaboratively with medical and bedside nursing staff to respond to an acute change in patient status, including an adult medical alert.
The provider performs comprehensive health assessments and manages patients with acute health problems.

- Implements the policies and procedures of the practice; performs assessment and reassessment of the patient according to applicable department standards and identified tools

- Writes orders and progress notes as indicated
- Assesses and manages common health problems
- Orders and evaluates pertinent diagnostic tests including but not limited to: radiology, laboratories and ECGs
- Prescribes pharmacological agents and non-pharmacological treatments for appropriate medical conditions
- Educates patients and/or families regarding disease process, medical intervention and health promotion
- Documents patient teaching and patient discharge summaries as needed
- Refers and communicates pertinent abnormalities to primary medical team
- Serves as a resource for nursing and ancillary staff regarding patient and disease management
- Addresses any clinical patient care issues, concerns and follow up as needed
- Actively participates in sepsis reduction program
- Other duties as assigned
