You will be providing Doc Go services in New Jersey by monitoring, assessing, and providing directed treatment common to complex injuries and illnesses through comprehensive assessments, including diagnosis, treatment, and outcome management for adult and pediatric patients. You will practice under direction with synchronous telemedicine support.
Day‑to‑Day Activities- Drive to patients’ homes to provide care and monitoring.
- Engage in assessments and provide treatment under the supervision of an Advanced Practice Provider (APP) for patients facing barriers to accessing the traditional healthcare system.
- Maintain patient medical records and documentation consistent with state regulations, Doc Go standards, and policy through our HealthPoint and Athena EMR system.
- Practical application of telemedicine support during care.
- Act as a conduit for the medical treatment of the Health Plan Partnerships patient population.
- Navigate conversations related to management of health, well‑being, medication management, and goals of care.
- Provide supervised, directed treatment of common medical diagnoses.
- Participate in continuing education as required by the state where you reside and the Doc Go training team.
- Adhere to clinical and safety standards, protocols, and performance metrics.
- Provide care with cultural sensitivity and compassion.
- Lead your practice with a patient‑centered approach.
- Other related duties as deemed necessary.
- Current unrestricted NJ State LPN license.
- Phlebotomy experience.
- Minimum of two years of experience as a Licensed Practical Nurse (LPN).
- Active and current American Heart Association BLS certification.
- Valid Driver’s License in state of residency with 2+ years of driving experience and an acceptable driving record.
- Ability to lift 50–60 lbs.
- Experience in Emergency Department, Urgent Care, Internal Medicine, Hospital – Critical Care, Family Practice or other relevant areas (preferred).
- Clinical work experience with pediatrics (preferred).
- Fluent bilingual English/Spanish (preferred).
