JOB SUMMARY:

The Clinical Coordinator RN, Radiation Oncology is a professional Registered Nurse responsible for coordinating, monitoring, and evaluating the care of patients receiving radiation oncology services. Working collaboratively under the direction of physicians and within an interdisciplinary care team, this role provides clinical oversight and care coordination for a designated patient population throughout the continuum of treatment.

The Clinical Coordinator RN serves as a key patient and family advocate by assessing clinical needs, establishing goals and expected outcomes, facilitating education, and ensuring seamless coordination of outpatient services. This role supports high-quality, patient-centered care while addressing the unique physical, emotional, and educational needs of patients across the lifespan, including pediatric, young adult, adult, and geriatric populations.

QUALIFICATIONS:
  • Graduate of an accredited School of Nursing required
  • Bachelor of Science in Nursing (BSN) preferred
  • Current Registered Nurse (RN) license in the State of New Jersey required
  • Minimum of 2 to 3 years of clinical nursing experience required
  • Oncology and/or Radiation Oncology experience preferred
  • Current Basic Life Support certification required
  • Strong understanding of care coordination, patient education, and outpatient clinical workflows
  • Demonstrated ability to communicate effectively with interdisciplinary teams, patients, and families
  • Excellent organizational, critical thinking, and care management skills
  • Ability to provide compassionate, patient-centered care in a fast-paced clinical environment
ESSENTIAL FUNCTIONS:

Patient Care Coordination
  • Coordinate, monitor, and evaluate care for a designated caseload of radiation oncology patients in the outpatient setting
  • Conduct comprehensive patient assessments including review of medical history, current condition, and ongoing care needs
  • Establish patient and family goals, monitor outcomes, and revise care plans as needed
  • Collaborate with physicians and interdisciplinary teams to ensure continuity and coordination of care
Patient & Family Education
  • Assess and implement patient and family education plans related to treatment, symptom management, and ongoing care needs
  • Identify and coordinate home care and supportive service needs when appropriate
  • Serve as a resource and advocate for patients and families throughout the treatment journey
Clinical Assessment & Care Management
  • Monitor patient progress and identify changes requiring intervention or escalation of care
  • Facilitate communication among care team members to support timely and effective patient management
  • Ensure documentation accurately reflects care coordination activities and patient progress
Age-Specific Care Delivery
  • Apply principles of growth and development to assess and address age-specific needs of patients across the lifespan
  • Deliver care and education tailored to pediatric, young adult, adult, and geriatric populations
  • Ensure care plans reflect individualized patient needs and treatment goals
Quality & Professional Practice
  • Maintain compliance with hospital policies, nursing standards, and regulatory requirements
  • Support quality improvement initiatives and evidence-based practice within Radiation Oncology services
  • Participate in ongoing professional development and continuing education activities
