The Licensed Practical Nurse (LPN) – Medical/Surgical provides direct and indirect patient care under the supervision of a Registered Nurse. The role includes monitoring vital signs, administering medications, performing wound care, and assisting with activities of daily living. The LPN ensures patient safety, adherence to care plans, and optimal outcomes in accordance with established nursing standards and hospital policies.
Essential Functions- Provides direct patient care, including monitoring vital signs and assisting with ADLs
- Administers medications as prescribed within scope of practice
- Documents patient care, assessments, and interventions accurately and timely
- Assists with specimen collection, wound care, and other clinical procedures as directed
- Collaborates with the RN in developing, updating, and implementing patient care plans
- Monitors and reports changes in patient condition to the RN or physician
- Provides patient and family education regarding care plans and discharge instructions
- Maintains a safe and clean patient environment in accordance with infection control protocols
- Participates in staff training, competency assessments, and quality improvement initiatives
- Responds to emergencies and assists in patient stabilization as directed
- Knowledge of basic nursing principles, medication administration, and patient care procedures
- Strong communication and interpersonal skills for interacting with patients and the healthcare team
- Ability to work effectively in a fast-paced, high-acuity environment
- Proficiency in electronic medical records (EMR) systems
- Demonstrates professionalism, accountability, and a commitment to patient-centered care
- Ability to lift and move patients and equipment as necessary
- Strong organizational skills and attention to detail
- Graduate of an accredited Practical Nursing Program required
- Current state LPN/LVN licensure required
- Basic Life Support (BLS) Certification as required per facility policy
- De-escalation Training as required per facility policy
- Minimum of six (6) months of experience in a medical/surgical setting preferred
