MSW hospice part time

We are seeking a compassionate and dedicated MSW Hospice Part Time professional to join University Home Hospice Inc in Livonia, MI. This role involves providing emotional support, counseling, and resource coordination to patients and their families during end-of-life care. The ideal candidate will have strong interpersonal skills and a commitment to delivering high-quality hospice care.

• Conduct psychosocial assessments of patients and families to identify needs and develop care plans.
• Provide counseling and emotional support to patients and their families facing terminal illness.
• Collaborate with interdisciplinary hospice care teams to coordinate patient care and resources.
• Assist with discharge planning and link patients and families to community resources.
• Document patient interactions and care plans accurately and timely.
• Participate in team meetings and quality improvement initiatives.

• Master’s Degree in Social Work (MSW) or LLMSW from an accredited program.
• Current state licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
• Experience in hospice or palliative care preferred.
• Excellent communication and interpersonal skills.
• Ability to work part-time hours in Livonia, MI.
