Starting Rate: $20‑22/hour

Position: Care Center Coordinator (Full Time)

What Makes This Job Unique

The Care Center Coordinator is responsible for ensuring Angela Hospice delivers excellent customer service for end‑of‑life patients. The Care Center Coordinator is responsible for providing daily operational and administrative support to the Care Center ensuring quality, consistency and continuity of hospice services. The Care Center Coordinator is committed to the philosophy, goals, and objectives of Angela Hospice and the Felician Franciscan Core Values.

Responsibilities
  • Works in close collaboration with the Lead Nurses and Care Center Leadership to establish, implement and evaluate goals and objectives for the Care Center
  • Participates in facilitation of patient services by supporting administrative, clerical, and clinical support operations in collaboration with the Lead Nurses and Care Center Leadership
  • Prepares schedule templates and schedules staffing changes and time‑off approval and coverage staying within budgeted staffing hours and following policies and procedures
  • Compiles and posts monthly schedules for Care Center clinical staff making adjustments as needed to provide adequate staffing for the Care Center
  • Attends Care Center meetings to keep current on expectations and problem areas as needed
  • Participates in auditing and preparing various reports on Care Center patient electronic charts and time‑keeping systems as requested/needed
  • Reviews admission attributes for correct level of care and updates as needed
  • Collaborates with the Informatics department as needed
  • Provides time‑keeping system support and education surrounding timesheets/visits
  • Integrates new staff into the Care Center and creates precepting schedules
  • Processes electronic payroll: verifies hours worked, accuracy of pay, enters stipends and necessary documentation included prior to submitting time sheet to payroll department
  • Maintains functionality of communication tools to Volunteer Board assignment sheets for each wing
  • Creates instruction manuals/guides as needed and assists with computer training or trouble‑shooting with time‑keeping systems and EMR
  • Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies
  • Fulfills the obligation of requested and/or accepted assignments
  • Supports department operations as directed as well as being assigned to any special projects
  • Performs tasks which are supportive in nature to the essential functions of the position, but which may be altered based upon the need of the organization
  • Meets ongoing requirements to remain compliant with Federal/State and/or other regulatory guidelines
  • Performs other duties as directed by the President/CEO, Director of Inpatient Services or other assigned designee
Qualifications
  • Associate’s degree required, Bachelor’s preferred
  • 1 year of healthcare experience required, home health and hospice experience preferred
  • Excellent organization and time‑management skills
  • Excellent reading, writing, and grammar skills
  • Proficient computer knowledge and skill:
    Microsoft Office and EMR’s
  • Proficient typing skills required
  • Excellent communication skills, both verbal and written, problem solving, and interpersonal skills
