Hospice RN

University Home Care Inc is seeking a compassionate and dedicated Hospice RN to provide exceptional end-of-life care to patients and their families throughout Wayne,Macomb, and Oakland Counties. This role involves coordinating and delivering nursing care that supports patients' comfort and dignity during their hospice journey.

• Provide comprehensive nursing care to hospice patients in their homes across Wayne, Macomb, and Oakland Counties.
• Assess patient conditions and develop individualized care plans in collaboration with the interdisciplinary team.
• Manage symptoms and administer medications according to physician orders.
• Offer emotional support and education to patients and their families regarding disease progression and comfort measures.
• Maintain accurate and timely documentation of patient care, assessments, and communications.
• Collaborate with physicians, social workers, and other healthcare professionals to ensure holistic patient care.
• Respond promptly to patient and family needs, including crises and changes in condition.

• Registered Nurse (RN) license in the state of Michigan.
• Minimum of 2 years nursing experience preferred, with hospice or home health experience highly desirable.
• Strong clinical assessment and critical thinking skills.
• Excellent interpersonal and communication skills.
• Ability to work independently and manage multiple patient cases effectively.
• Reliable transportation and willingness to travel within Wayne, Macomb, and Oakland Counties.
• CPR certification and any hospice-related certifications are a plus.
