About the Role
As a Hospice RN Case Manager, you will provide skilled nursing care and compassionate support to patients and families during life's final chapter. You will coordinate comprehensive care for your caseload while collaborating with physicians, facilities, and other interdisciplinary team members to address the physical, emotional, and spiritual needs of those in your care.
Responsibilities- Provide skilled nursing care and assess patients’ needs in home or facility settings.
- Coordinate and implement care plans with interdisciplinary team members.
- Maintain documentation and meet regulatory and organizational deadlines.
- Offer emotional support and education to patients and families.
Office:
Bingham Farms, MI
Service area:
Wayne County, Livonia, and surrounding areas. Requires daily travel within this area to provide care in patients’ homes, assisted living facilities, or nursing homes.
- Monday to Friday, 8:00 a.m. to 5:00 p.m.
- Weekend on‑call rotation
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current state license as a Registered Nurse.
- Minimum one (1) year experience as a nurse in a clinical care setting.
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage.
- Bachelor of Science in Nursing (BSN).
- Hospice or home health experience.
- Proficient nursing skills as defined by accepted nursing standards.
- Ability to assess and respond to the needs of patients, families, and caregivers in varied settings.
- Skill in coping with family emotional stress and tolerance of individual lifestyles.
- Ability to handle multiple priorities, documentation requirements, and deadlines.
$80,000—$90,000 USD
