Job Description

Registered Nurse RN After Hours/On Call Visit (Home Health) – After‑hours on‑call registered nurse providing care to patients in their homes.

Schedule:

7 days on, 7 days off; full time, salaried position. Must be available from 2:00pm to 8:00am for on‑call coverage and some scheduled visits during working weeks. Coverage areas include greater Lansing and St.Johns, up to Mt.Pleasant.

Responsibilities
  • Assess assigned case load of home health care patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, call‑in logs, and on‑site evaluations.
  • Make the initial evaluation and re‑evaluate the patient’s nursing needs during each visit.
  • Communicate significant findings, problems, and changes in health conditions, environment, or unsafe facility conditions to the clinical supervisor, the physician and/or other personnel involved with patient care.
  • Counsel, instruct, and implement the plan for patient safety, utilizing patient information, family, and community resources.
  • Participate in the implementation of the plan of care for an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes.
  • Identify deviations from normal nursing and patient condition status.
  • Initiate appropriate actions based on deviations.
  • Verify the Plan of Care prior to each visit and provide care according to physician’s orders, assessment data, and established standards and guidelines.
  • Perform other duties/projects as assigned.
What is Required?
  • Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
  • 1+ year of experience as a nurse in a clinical care setting.
  • Ability to frequently lift, push, pull, and support up to 50pounds, including positioning or transferring patients and moving equipment.
  • Home Health and OASIS experience is strongly preferred, but not required.
  • Current State license as a Registered Nurse.
  • Ability to travel approximately 50%.
  • Excellent verbal and written communication skills.
