In-Home Physician Part Time - Kalamazoo

Signify Health, part of CVS Health, seeks a Part Time Physician to provide In-Home Health Evaluations, engaging with people in their homes and helping bridge gaps in care. You’ll build trusted relationships to support healthier communities by delivering flexible, comprehensive, and personalized health evaluations at people’s front doors.

• Provide in-home health evaluations that typically last about 45 minutes, including a health history review, physical evaluation, and any diagnostic tests ordered by the member’s health plan (e.g., diabetic eye exams, spirometry).
• Engage one-on-one with patients, answering health questions and connecting them to appropriate care within their plan.
• Utilize an iPad and a clinical workflow to document assessments; you are not required to prescribe medications, order labs, or alter a patient’s current treatment regimen.
• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as needed and provide all Signify Health services as indicated, including diagnostic and preventive services (e.g., spirometry, bone density screening).
• Collaborate with Signify Health clinicians as part of a national network focused on improving health outcomes.

• Active, unrestricted medical license(s) in the coverage area(s).
• Multi-state licenses (preferred).

The primary workplace is the private residence of health plan members. Environments may vary and can include households with pets, secondhand smoke, or other potential allergens or irritants. You must be able to perform duties safely in homes that may present these conditions.

The typical pay range for this role is $105.00 per hour. The actual base salary offer depends on experience, education, geography, and other factors.
