In-Home Nurse Practitioner or Physician Assistant Part Time - Lansing

Job Overview

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations on a flexible, part‑time schedule and help bridge gaps in care.

• Conduct in‑home visits lasting approximately 45 minutes, including medication and medical history review, physical evaluation, and optional diagnostic tests such as diabetic eye exams or spirometry.
• Use an iPad and an intuitive clinical workflow for each evaluation; no prescribing, order of lab tests or alteration of current treatment within the visit.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).
• Collaborate with Signify Health colleagues, travel locally and within licensed states as needed, and conduct virtual visits when required.
• Build trusted relationships with patients, assist them in connecting to the right care and improve overall health outcomes.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Valid NP or PA credentials.

• Primary workplace is the private residence of health plan members.
• Must safely perform duties in homes that may contain household pets, secondhand smoke, or other airborne irritants.

Pay Range: $41.85 – $90.13 per hour. Anticipated Weekly
