In-Home Traveler Nurse Practitioner or Physician Assistant - Michigan

Join CVS Health® through Signify Health as a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to deliver in‑home health evaluations across a defined coverage area. The role involves extensive travel—up to 100% of the time—and may require overnight stays, car rentals, or flights. Multiple state licenses may be necessary.

• 45‑minute home visits that include medication and medical history review, a physical evaluation, and selected diagnostic tests if ordered (e.g., diabetic eye exam or spirometry).
• Use of an iPad and an intuitive clinical workflow; prescribing medication, ordering lab tests, or altering a patient’s treatment plan is not required.

• Provide in‑home health evaluations, building trusted relationships with patients.
• Travel locally and statewide to meet patients within the licensed coverage area.
• Conduct virtual visits as needed.
• Deliver all Signify Health diagnostic and preventive services products (spirometry, DEE, bone density screening, etc.).

Primary workplace is the private residence of health‑plan members. Clinicians must be able to work safely in environments that may contain pets, secondhand smoke, and other potential allergens or irritants.

• Active, unrestricted NP or PA license(s) in the coverage area(s).

The typical pay range for this role is $87,035.00–$, based on experience, education, geography, and other relevant factors.

Anticipated weekly hours: 40 hours per week (full time).
