Trinity Health Alliance of Michigan is seeking an experienced RN Care Manager in Southeast Michigan for an in-person, full-time role. You will perform assessments, develop care plans, coordinate with physicians, and monitor high-risk members across the continuum of care with the goal of reducing inpatient readmissions.
Ideal candidates have a BSN, an active Michigan RN license, CPR certification, and at least three years of acute care or case management experience.
