This is a full-time on-site role located in Michigan for a Home Health Nurse. The Home Health Nurse will provide skilled nursing care to patients in their homes, including medical assessments, wound care, administering medications, and patient education. Additional responsibilities include coordinating with interdisciplinary healthcare teams to develop and implement care plans, ensuring holistic and patient-centered care. The nurse will also educate patients and families on health management, while maintaining accurate documentation of patient progress and treatment plans.

• Proficiency in Nursing, Home Care, and Patient Care skills
• Experience and expertise in Wound Care and Medicine
• Strong organizational and time-management abilities to manage multiple patients and coordinate care services
• Excellent interpersonal and communication skills for patient and family education
• Licensure as a Registered Nurse (RN) in the state of Illinois
• BLS (Basic Life Support) certification required
• Experience in home health or community-based nursing is preferred
