• Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Monitor and report changes in patient condition to the registered nurse or other healthcare professionals
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care delivery
• Participate in quality improvement initiatives to enhance patient outcomes and resident satisfaction
