Responsibilities:

• Develop and implement individualized education plans for residents and their families, focusing on disease prevention, symptom management, and overall health promotion
• Provide education and support to residents on medication management, self-care, and disease management
• Collaborate with interdisciplinary team members to develop and implement evidence-based interventions and treatment plans
• Conduct patient assessments and evaluations to identify learning needs and preferences
• Design and deliver educational programs and workshops to staff, residents, and families on various topics related to health and wellness
• Monitor and evaluate the effectiveness of education programs and make recommendations for improvement
• Stay up-to-date with industry trends and best practices in patient education and continues to advance knowledge and skills

Requirements:

• Current licensure as a Registered Nurse in the state of Pennsylvania
• Strong communication and interpersonal skills
• Ability to work effectively with diverse populations, including residents, families, and healthcare professionals
• Strong organizational and time management skills
