As a Registered Nurse 7 to 3 RN, you will be responsible for delivering high-quality patient care in our post-acute and long-term care settings. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patient progress, and provide education and support to patients and their families. You will also be responsible for communicating effectively with patients, families, and healthcare professionals to ensure seamless care transitions.

• Provide direct patient care, including administering medications, wound care, and treatment interventions
• Develop and implement individualized care plans to meet patients' unique needs
• Monitor patient progress, identify areas for improvement, and adjust care plans accordingly
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
• Provide education and support to patients and families to promote optimal health outcomes
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
• Participate in quality improvement initiatives to enhance patient care and outcomes
