Shift: Day Shift (8:00 AM – 6:30 PM)
Hours: 40 hours/week
Shift Length: 10 hours
Duration: 13 weeks

What You’ll DoLead coordination of patient care across interdisciplinary teams
Monitor care plans for effectiveness, timeliness, and appropriateness
Collaborate with physicians on treatment plans, length of stay, and discharge strategies
Serve as a liaison between patients, families, and care providers
Focus on complex populations (chronic conditions, high utilizers, etc.)
Coordinate transfers and ensure continuity of care
Support regulatory compliance and data reporting
RequirementsEducation & Experience:
BSN (preferred)
Minimum 1 year acute care case management experience
Licensure & Certifications:
Active RN license (required)
Case Management Certification (preferred)
