We are seeking an experienced RN Case Manager to join a collaborative care management team. This role is essential in coordinating patient care across the continuum; from pre-admission through post-discharge; ensuring optimal clinical outcomes, efficient resource utilization, and a seamless patient experience.

You will serve as a key liaison between patients, families, physicians, and multidisciplinary teams while guiding care planning and discharge processes.

• Coordinate and oversee patient care plans across disciplines, ensuring timely and appropriate care
• Collaborate with physicians on treatment plans, length of stay, and discharge planning
• Monitor diagnostics, treatments, and procedures to align with clinical and coverage guidelines
• Act as a liaison between patients, families, care teams, and payors
• Focus on complex cases, including frequent ED utilizers, chronic conditions, substance use, and vulnerable populations
• Facilitate transfers to tertiary care centers and ensure continuity of care
• Support compliance with regulatory and accreditation standards
• Analyze trends in frequent admissions and contribute to care improvement strategies

• Mandatory
  • Active RN license (state of practice)
  • Minimum 1 year of acute care case management experience
• Preferred
  • Bachelor of Science in Nursing (BSN)
  • Case Management Certification (CCM or equivalent)
  • Prior experience in a care management role
