Position OverviewWe are seeking an experienced RN Case Manager to join a collaborative care management team. This role is essential in coordinating patient care across the continuum; from pre‑admission through post‑discharge; ensuring optimal clinical outcomes, efficient resource utilization, and a seamless patient experience. You will serve as a key liaison between patients, families, physicians, and multidisciplinary teams while guiding care planning and discharge processes.
Key Responsibilities- Coordinate and oversee patient care plans across disciplines, ensuring timely and appropriate care
- Collaborate with physicians on treatment plans, length of stay, and discharge planning
- Monitor diagnostics, treatments, and procedures to align with clinical and coverage guidelines
- Act as a liaison between patients, families, care teams, and payors
- Focus on complex cases, including frequent ED utilizers, chronic conditions, substance use, and vulnerable populations
- Facilitate transfers to tertiary care centers and ensure continuity of care
- Support compliance with regulatory and accreditation standards
- Analyze trends in frequent admissions and contribute to care improvement strategies
- Active RN license (state of practice)
- Minimum 1 year of acute care case management experience
- Bachelor of Science in Nursing (BSN)
- Case Management Certification (CCM or equivalent)
- Prior experience in a care management role
