Roles & Responsibilities:

• Provide exceptional patient care by assisting in the development, implementation, and revision of the case management program.
• Act as a liaison between patients, families, healthcare providers, and the multidisciplinary team to optimize care outcomes.
• Coordinate the continuum of care for patients from pre-admission through post-discharge.
• Ensure that patients and families navigate smoothly through the hospitalization process.
• Serve as a consultant to the healthcare team regarding patient needs.
