• Ensure compliance with physician orders, timely visit frequency, and accurate documentation of progress toward discharge goals.
• Follow policy, procedure, and standard of care for assessment, planning, intervention, and evaluation; provide holistic care to patients and significant others.
• Document clinical judgments, critical thinking, and ethical decisions in the EMR system.
• Manage payer authorizations, maintain coverage guidelines, and communicate with insurance nurses as required.
• Complete nursing assessments, medication reconciliation, and collaborate on the Plan of Care; update the plan within two weeks of patient status changes.
• Implement teaching strategies that align with care goals; reflect those strategies in the chart.
• Report care problems to management and physicians in a timely manner with suggested solutions.
• Act as liaison among physicians, Clinical Practice management, and the health‑care team to coordinate resources.
• Communicate patient information and the Plan of Care during transfers or when scheduling time off.
• Complete routine assignments efficiently and independently, meeting productivity requirements.
• Stay current with evolving standards and incorporate them into practice; document knowledge of updated guidelines.
• Perform other duties and projects as assigned and demonstrate flexibility.
• Adhere to HMH organizational competencies and standards of behavior.
• Lift and pull a minimum of 50 lbs and stand a minimum of eight hours a day.

• Associate’s Degree in Nursing or Nursing Diploma/Certificate.
• Minimum of one (1) year of RN experience.
• Experience with inserting IVs.
• Competency in IV insertion and vascular access: demonstrate with a preceptor inserting peripheral line, accessing ports, PICCs, and central lines within one year; or complete a VAD course and practicum if not within one year.
• ONS/ONCC Chemotherapy/Immunotherapy Certificate within three (3) months of hire.
• Successful completion of Infusion Test in probationary period with a grade of 80 % or higher.
• Excellent written and verbal communication skills.
• Proficient in Google Suite and/or Microsoft Office.

• BSN

• New Jersey State Professional Registered Nurse License.
• AHA Basic Health Care Life Support HCP Certification.
