About the Role
Back @ Work Physical Therapy, a brand partner of Upstream Rehabilitation, is looking for a Full Time Patient Care Coordinator to join our team in Logan, UT.
Position SummaryThe Patient Care Coordinator - I (PCC-I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures and support the mission, vision, and values of Upstream Rehabilitation.
A day in the life- Greets everyone who enters the clinic in a friendly and welcoming manner.
- Schedules new referrals received by fax or telephone from patients or physician offices.
- Verifies insurance coverage for patients.
- Collects patient payments.
- Maintains an orderly and organized front office workspace.
- Other duties as assigned.

- Collect money due at the time of service.
- Convert referrals into evaluations.
- Schedule patient visits.
- Customer Service – greet, create an inviting clinic atmosphere, make welcome calls, monitor and influence arrival rate through a great customer experience.
- Manage schedule efficiently.
- Manage document routing and personal overtime.
- Manage non-clinical documentation, deposits, caseload, D/C candidate, progress note, and insurance reporting.
- Monitor clinic inventory.
- Attend required training with Territory Field Trainers for Raintree and other business process updates.
- Complete quarterly compliance training.
- High School Diploma or equivalent.
- Strong communication skills – able to relate well with Business Office and Field leadership.
- Ability to multitask, attention to detail, meet deadlines, work with little or no supervision.
- Team member with efficient time management and presentation skills.
- Position subject to inside environmental conditions; exposure to noise consistent with indoor environment.
- Full-time duty Monday through Friday for minimum 40 hours per week; may be required to attend special events, evenings, weekends, or work additional hours.
- Primarily sedentary work with periodic lifting of 0-20 lbs.
- Constantly sits, sits and stands as needed, uses computer, phone, printer, keyboard, mouse, copy machines.
- Engages in repetitive typing, computer screen focus, comprehension of information, and verbal communication.
- Occasional walking, standing, pushing or pulling up to 20 lbs.; rarely crawling, crouching, kneeling, stooping, climbing stairs or ladders, reaching above shoulder height, lifting under 10 lbs. waist to shoulder.
