Speech Language Therapist (SLT) - Home Health

Speech-Language Therapist (SLT) – PRNAscend Home Health & Hospice | Northern Utah Ascend Home Health & Hospice is seeking a compassionate and motivated Speech-Language Therapist (SLT) to join our growing team on a PRN basis serving patients throughout Cache and Box Elder Counties in Northern Utah.

This position offers flexibility and the opportunity to make a meaningful impact by helping patients improve communication, cognition, and swallowing abilities in the comfort of their own homes.

Responsibilities:

Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders

Develop individualized treatment plans based on patient needs and goals

Educate patients, families, and caregivers on therapy techniques and care plans

Collaborate with physicians, nurses, therapists, and the interdisciplinary care team Complete timely and accurate documentation

Qualifications:

Current Utah SLT license required

Home health experience preferred, but not required

Strong communication and organizational skills

Compassionate, dependable, and patient-focused

Valid driver's license, reliable transportation, and current auto insurance required

Position Details:

PRN / Flexible scheduling

Coverage area includes Cache and Box Elder Counties Pay per visit DOE (dependent on experience)
