Position Overview
We are seeking a compassionate, skilled, and dedicated Licensed Practical Nurse (LPN) to join our team at Terrace Grove Assisted Living, an assisted living community committed to providing exceptional care to our residents. As an LPN, you will play a vital role in ensuring the well-being, comfort, and safety of our elderly residents while working closely with registered nurses (RNs) and other healthcare professionals.
This position offers the opportunity to make a meaningful difference in the lives of older adults in a supportive, team‑oriented environment.
- Resident Care: Provide direct care to residents, including monitoring vital signs, administering medications, and assisting with activities of daily living (ADLs) such as bathing, dressing, eating, and mobility.
- Medication Management: Administer prescribed medications, including oral, topical, and injections, in accordance with physician orders and facility policies. Monitor for side effects and document appropriately.
- Health Monitoring: Regularly assess residents' physical and mental health, recognizing changes in condition. Report concerns to the RN or physician and assist with ongoing care planning. Maintain accurate and timely documentation of patient care, medication administration, and observations in electronic health records (EHR). Ensure compliance with Utah regulations and facility policies.
- Care Plan Support: Participate in the development and revision of individual care plans in collaboration with the nursing team, physicians, and families. Ensure that care plans are followed and residents' needs are met. Act as a liaison between the residents, families, and the healthcare team, providing updates and addressing concerns regarding resident health and well‑being.

Work with nursing staff, physicians, therapists, and other healthcare professionals to ensure comprehensive care and a cohesive approach to resident well‑being. - Compliance & Safety: Adhere to all safety protocols, infection control standards, and regulatory guidelines. Ensure the facility meets all Utah and federal nursing standards.
- Licensure: Current and valid LPN license in the state of Utah.
- Education: Graduation from an accredited practical nursing program.
- Skills & Abilities:
- Strong knowledge of nursing principles, practices, and procedures.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate, patient, and empathetic demeanor with the ability to build trust with residents and families.
- Strong organizational skills and attention to detail.
- Ability to handle medical emergencies calmly and efficiently.
- Ability to lift up to 50 pounds and assist residents with mobility as needed.
- Ability to stand, walk, bend, and stoop for extended periods.
- Ability to respond to medical emergencies, including performing CPR and first aid.
- This position will involve working in a long‑term care facility setting, which includes resident rooms, common areas, and healthcare offices.
- Shift work, including evenings, weekends, and holidays, may be required based on facility needs.
