Position Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will play a key role in delivering direct nursing care, monitoring patient health, and collaborating with interdisciplinary staff to ensure the highest standards of care.

Key Responsibilities

* Provide direct patient care under the supervision of a Registered Nurse (RN) or physician

* Administer medications and treatments as prescribed

* Monitor and record patient vital signs and conditions

* Assist with daily living activities and ensure resident comfort

* Maintain accurate and timely documentation of patient care

* Communicate effectively with residents, families, and healthcare team members

* Respond promptly to changes in patient conditions or emergencies

* Ensure compliance with all healthcare regulations and facility policies

Qualifications

* Current, active LPN license in the state of employment

* CPR/BLS certification (or willingness to obtain upon hire)

* Previous experience in a skilled nursing or long-term care setting preferred

* Strong clinical, organizational, and communication skills

* Compassionate, patient-focused approach to care

* Ability to work collaboratively in a team environment
