Overview
Registered nurse plans, organizes and directs home care services. Experience in nursing with emphasis on community health education.
The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Job TitleRegistered Nurse (RN)
Responsibilities- Complete initial assessment of patient and family to determine home care needs.
- Provide a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluate patient nursing needs.
- Initiate the plan of care and make necessary revisions as patient status changes.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions.
- Include the patient and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician.
- Provide direct patient care as defined in the State Nurse Practice Act.
- Counsel patient and family in meeting nursing and related needs.
- Provide health care instructions appropriate per assessment and plan of care.
- Identify discharge planning needs and implement prior to discharge.
- Act as case manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload.
- Prepare clinical notes and update primary physician at least every 60 days.
- Communicate with physician regarding patient needs and report changes; obtain orders as required.
- Communicate with community health related persons to coordinate the care plan.
- Participate in on-call duties as defined by the on-call policy.
- Ensure arrangements for equipment and other necessary items and services are available.
- Instruct, supervise and evaluate home health aide care provided every two weeks.
- Graduate of an accredited school of nursing.
- One to two years of recent acute care experience in an institutional setting.
- Current licensure in state and CPR certification.
- Bachelor’s degree, with one year of home health care experience preferred.
- Management experience not required; responsible for supervising home health aides.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, and nursing skills per competency checklist.
- Physical ability: prolonged walking or standing, lifting/positioning/transfer patients, lifting supplies and equipment, reaching, stooping, bending, kneeling or crouching.
- Visual acuity and hearing sufficient to perform required nursing skills.
- Licensed driver with automobile in good working order and insured per organization requirements.
