RN Home Health
We are currently seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide exceptional healthcare services to patients in their homes. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for delivering personalized care to individuals in need. This is a part-time position. We make every effort to accommodate your personal schedule.
Responsibilities:
- Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors.
- Develop and implement individualized care plans based on assessment findings and in collaboration with the patient, their family, and other healthcare team members.
- Provide skilled nursing care, including wound care, medication management, infusion therapy, and other treatments as prescribed by the physician.
- Monitor patients' progress and response to treatment, and make necessary adjustments to the care plan.
- Educate patients and their families about their medical conditions, treatment options, and self-care techniques to promote optimal health and independence.
- Coordinate care with other healthcare professionals, including physicians, therapists, social workers, and home health aides, to ensure continuity of care and comprehensive support for patients.
- Document all patient assessments, interventions, and outcomes accurately and in a timely manner, adhering to regulatory and organizational standards.
Qualifications:
- Valid Registered Nurse (RN) license in the state of Utah.
- BLS certification required; ACLS certification preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and interdisciplinary team members.
- Knowledge of Medicare regulations and documentation requirements for home health services.
- Ability to work independently and prioritize tasks effectively in a fast-paced environment.
- Reliable transportation and willingness to travel to patients' homes.
- Valid driver's license and proof of insurance.
