Position: Case Manager (Receiving Center)

Seeking Case Managers to start mid-June 2026.

We are hiring for part time and full time Swing (3pm - 11pm) and Grave (11pm - 7am) shifts. Please specify which shift you can work when you apply.

Job Description
  • Provide immediate, short-term care and recovery supportive services for adult individuals experiencing mental health or substance use crises. Provide rapid‑care coordination to effectively de‑escalate and stabilize the individual, facilitating crisis interventions, if needed. Provide observation of Receiving Center clients. Collaborate with a multidisciplinary planning team for a needs assessment and service plan. Connect patients to necessary medical, mental health, and other available community services, ensuring a safe and efficient transition to lower levels of care.

    Communicate with family members and support systems (when appropriate ROI is in place) to provide education and ensure a continuum of care/support, and maintain independent recovery upon discharge. Provide transportation to appropriate services, as needed. Responsible for light meal prep and laundry services. Other duties and procedures as specified by the supervisor.
  • Help clients gain access to needed medical, social, or educational services and teach them and their families how to access community services to better manage mental and substance use disorders. Assess clients' status/functioning as it relates to the above services. (History, input from family, medical providers, social workers, educators, etc.) Complete Needs Assessments (NA) that identify areas of service need, with input from clients and others who participated in the Needs Assessments.

    Link clients to needed community resources identified in the Service Plan. Assist clients in integrating their behavioral, social, educational, dental, and physical health care needs by facilitating attendance at appointments, communicating with health care providers, assisting clients in understanding health care instructions and information; obtain/maintain eligibility for entitlements. Coordinate the ongoing delivery of services to clients with various agencies/providers clients have been linked with.

    Contact family members for feedback and alert them to changes in status/needs when appropriate releases are in place. Facilitate regular contact with behavioral, social, educational, dental, and physical health care providers, as needed, to help clients obtain any needed level of care. Instruct clients/caretakers in independently accessing needed community services. Monitor/follow‑up of behavioral, social, educational, dental, and physical health care needs with non‑BRMH providers of shared clients that need further care or have ongoing behavioral, social, educational, dental or physical health care needs and ensure timely access, quality and effectiveness of service, symptomatology, the need for face‑to‑face care, and appropriate Service Plan implementation.

Provide services that treat specific "behavioral problems" using a psycho educational approach (stress management, relaxation techniques, assertiveness training, conflict resolution, behavior modification, etc.). Provide needed instructions about the importance of regularly attending to dental and physical health care needs, including timely appointments and specialty care, which may include role play, problem solving, etc.

Help clients regain or enhance basic living skills needed for living in the least restrictive environment possible by teaching socialization, interpersonal and communication skills, appropriate behaviors, medication compliance, grooming and hygiene skills, etc. in a psychosocial rehabilitative service or outpatient setting with the goal of restoring the client to his/her best possible functional level. Assist clients to eliminate or reduce symptoms of their diagnosis and improve personal motivation and self‑esteem.

Provide needed instructions about the importance of regularly attending to behavioral, social, educational, dental and physical health care needs, including timely appointments and specialty care.
