Physical Therapist (PT) - Full Time

Chase Center Rehabilitation and Nursing Services is seeking a dedicated and skilled Physical Therapist (PT) to join our in-house rehabilitation team. Whether you are looking for a consistent full-time schedule or the flexibility of PRN work, this role offers the opportunity to make a meaningful impact in a skilled nursing setting.
We are looking for a clinician who brings strong clinical expertise, professionalism, and a passion for helping residents regain mobility, confidence, and independence.
Responsibilities- Perform comprehensive physical therapy evaluations
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Deliver evidence-based therapeutic interventions
- Monitor resident progress, update goals, and adjust treatment plans as needed
- Accurately document evaluations, treatments, and outcomes in compliance with all regulatory requirements
- Collaborate with the interdisciplinary care team to optimize resident outcomes
- Educate residents, families, and caregivers on therapy goals, progress, and home exercise programs
- Current Physical Therapist (PT) license in the state of Indiana
- Graduate of an accredited Physical Therapy program
- Must be at least 18 years of age
- Skilled nursing or long-term care experience preferred
- Familiarity with Net Health/Optima documentation systems is a plus
