Advanced Practice Provider (Senior Care) – Nurse Practitioner or Physician Assistant

Full-time – 4 days on site, 1 admin day. High compensation opportunity.

Ascend Medical is seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to provide relationship-centered care at premier assisted living facilities in the Loganville area, Grayson, and Sugar Hill. This full-time role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity.

• Provide compassionate, relationship-based care to older adults in an assisted living environment.
• Build trust with patients, families, and facility staff through clear communication and consistent presence.
• Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient.
• Collaborate with the facility’s care team to ensure seamless, coordinated support.

• Maintain flexible availability for patient visits and on-site presence.

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary).
• Master’s degree from an accredited program.
• Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
• Active DEA license (or eligibility to obtain).
• Availability to work on-site in Loganville and surrounding area.
